In Network
If this is your insurance company, please know that only straight Medicare Part B covers the diagnoses of diabetes and chronic renal failure and only with a referral. If you have a Medicare supplemental insurance policy, we encourage you to contact them prior to your visit for clarification of coverage.
Medicaid

Please note we are only in-network with Partnership Health Plan of California. We cannot bill Partnership if they are your supplementary insurance. We encourage you to contact them prior to your visit for clarification of coverage.
United Healthcare

This insurance company will cover most services offered by a Registered Dietitian Nutritionists. Referral is required. If your plan is a Medicare managed program, medicare limits apply however. Please contact us if you need more information.

We are in network, however only certain diagnosis are covered. Please call your customer service number and inquire as to your medical nutrition therapy plan benefits. Referral from a health care provider is needed to bill this insurance.
